Acute Hyperventilation Syndrome mimicking Unstable Angina

Hyperventilation is already a known cause of transient T-wave inversion; but ST segment depression is infrequently seen in routine electrocardiogram of patients with hyperventilation Syndrome. A 35 year-old female patient presented in emergency department with complaint of acute onset breathlessness. ECG of patient shows dynamic ST-T changes which normalizes after two hour. Her blood tests were normal with negative troponin results .Her arterial blood gases showed respiratory alkalosis.This case is a reminder of an under- recognized physiological phenomenon involving the cardiac conduction during hyperventilation.
