Background: Febrile thrombocytopenia is a common clinical presentation in various infectious diseases. Predicting platelet recovery is crucial for patient management. This study aimed to evaluate the utility of immature platelet fraction (IPF) as a predictor of platelet recovery in patients with febrile thrombocytopenia. Methods: This prospective observational study included 60 adult patients with fever (≥38°C) and thrombocytopenia (<150,000/µL). IPF was measured daily using an automated hematology analyzer. Platelet recovery was defined as an increase to ≥50,000/µL or doubling of the baseline count. The correlation between IPF and time to platelet recovery was assessed, and receiver operating characteristic (ROC) analysis was performed to determine the optimal IPF cut-off for predicting recovery within 72 hours.
