Idiopathic hypereosinophilic syndrome (IHES) is a rare systemic disorder with blood eosinophilia and multiple system involvement. A 63-year-old man presented with numbness on the left arm and left leg, shortness of breath, atypical chest pain and abdominal pain. On the transthoracic echocardiography, biventricular thrombus were seen. Cardiac involvement occurs in more than 60% of patients and is the most common cause of morbidity and mortality in IHES patients. Computed tomography showed thrombi in left, right ventricle, multiple pulmonary arteries and mesenteric artery. Cardiac magnetic resonance imaging showed ventricular thrombus with subendocardial fibrosis. Cardiac involvment resolved with treatment but at follow up, restrictive filling pattern was fixed with echocardiography.
